Weston Road Animal Hospital is working to be a Fear Free Veterinary environment. With fear free certified and trained staff members, our goal is to minimize Fear, Anxiety and Stress (FAS) for our patients and clients. If you are a first time client, please complete one of the documents below and submit to our office  This will help expedite the registration process for your first appointment. We look forward to meeting you and your pet!  You can also print a copy to bring with you to your appointment.

OFFERING CURBSIDE SERVICE

Although we have re-opened the lobby and exam rooms to clients, Weston Road Animal Hospital is still offering curbside service to owners who request it. To minimize time on the telephone with our staff, please download and complete our patient history form at home before your pet's visit. This form should be given to our staff when they come get your pet from the vehicle.

Boarding Forms

If your pet will be boarding with us, please download and complete our boarding agreement to expedite the check in process for your pet's stay. A new boarding agreement must be submitted annually.
